چکیده
Background. Intra-abdominal hypertension complicates the course of secondary diffuse peritonitis in at least 60–70% cases. Abdominal decompression is an essential component complex therapeutic measures patients with peritonitis. However, no clear criteria for choosing method and timing abdominal have been developed so far. The effectiveness its use various forms remains controversial. Objectives — to study dynamics intra-abdominal pressure early postoperative period when using methods Methods. work was performed on basis observational clinical under auspices Regional Clinical Emergency Hospital over a from January 2021 December 2022. authors studied 74 period. In 39 (52.7%) (group 1), carried out form prolonged nasogastric intubation. 15 (20.3%) 2) underwent nasointestinal Group 3 represented by 10 (13.5%) which were affected open abdomen technology vacuum assisted closure (VAC). 4 included vacuum-assisted laparostoma who severity assessed WSES cIAIs Score Mannheim Peritonitis Index. determined through urinary catheter Uno Meter Abdo Pressure® Kit (Unomedical, Russia) before surgery within 5 days Statistical analysis based non-parametric Wilcoxon test comparing values same group different periods observation. Mann-Whitney U-test used compare absolute groups. Results. Nasogastric tube did not provide acceptable reduction pressure, especially severe Prolonged intubation contributed steady decrease throughout period, while simultaneous resulted increase after decline one day surgery. best results observed case laparostomy combination Throughout exceed 15–17 mmHg. This can be only treatment most peritonitis, indications sanitation relaparotomy threatened compression syndrome. Conclusion. data obtained enable process surgical tactics optimized respect hypertension.
